Feedback Form
If you require support to provide feedback, please speak to your practitioner who will provide you with all of your options and help you to engage independent support for the process.
You will receive an acknowledgement of your feedback within 2 business days. We will endeavour to resolve any issues within 14 business days. For escalation, please request a copy of our complaints process.
We value honesty and transparency and we are always open to feedback that helps
us to improve our policies and practices. Please provide your details if you would like to receive updates on the actions taken to address your feedback.
